Lea Yost

Lea is an experienced physical therapy practitioner, case management professional, and rehabilitation healthcare director with over 20 years of success in delivering and coordinating high quality care for patients in sub-acute, outpatient, and home health settings. She conducts patient evaluations, creates tailored care plans, delivers daily therapy, performs reassessments, uses standardized tests to track progress, and directs therapy to help patients achieve optimal function and quality of life. She is passionate about helping patients return to the daily activities they love.
