What to Expect

We offer the finest professional team, with diverse experience and interests combined with enthusiasm and compassion.

Please arrive to your initial evaluation 15 minutes early to ensure that all of your paperwork is completed and processed and remember to wear loose, comfortable clothing.

Please make sure to bring the following with you to this appointment:

  • Photo ID
  • Insurance card
  • Provider referral/prescription
  • The forms below (completed and signed)

You will be evaluated by a licensed physical therapist or occupational therapist which will include time for your therapist to get to know you so that they may better understand your injury. Each patient’s situation is unique and your plan of care will be set up just for you.

Missed Appointments:

If you can’t make your appointment, please call the clinic as soon as you know, so that you can reschedule your visit. We require 24 hour notice for the cancellation of a scheduled appointment.

Insurance/ Customer Service

Medical insurance generally covers physical and hand therapy treatment. Check with your insurance carrier to determine your coverage benefits. We are contracted with most major insurance companies and will be glad to answer questions you may have about your benefits.

Physician Referrals

You may receive care at IRG without a referral or prescription. However, your insurance plan may require a written physical referral. Our therapists are ready to work with you and your physician on your path to wellness.
